Dosage and administration

  • This drug can be taken without regard to meals, but it is recommended to take it at the same time each day.

    1. Hypertension

    • 1) Adults

      The usual starting and maintenance dose of Losartan potassium is 50 mg once daily. The maximal antihypertensive effect occurred in 3-6 weeks after initiation of therapy. If the therapeutic effect is insufficient, the dose can be orally administered in divided doses twice daily or if necessary, the dose can be increased to 100 mg once daily.

    • 2) Geriatric use

      The recommended starting dose is 25mg once daily in the elderly over 75 years old. but no dosage adjustment is necessary for patients under 75 years old.

    • 3) Patients with renal impairment
      • (1) creatinine clearances 20 ∼ 50 mL/min: No initial dose adjustment is necessary.
      • (2) creatinine clearances < 20 mL/min and hemodialysis patients: A starting dose of 25 mg once daily should be considered.
    • 4) In patients with intravascular volume depletion

      For patients with intravascular volume-depletion (e.g., those being treated with diuretics), a starting dose of 25 mg once daily should be considered (see ‘Warnings and precautions’ paragraph).

    • 5) Patients with hepatic impairment

      A lower dose should be considered for patients with a history of hepatic impairment. Losartan potassium is contraindicated in patients with severe hepatic impairment.

    • 6) Pediatric patients above 6 years of age and adolescents
      • (1) Patients weighing more than 20 kg but less than 50 kg who can swallow tablets

        The recommended dose is 25 mg once daily, and the dose can be adjusted to a maximum of 50 mg once daily according to blood pressure response.
      • (2) Patients weighing more than 50 kg

        The usual dose is 50 mg once daily, and the dose can be adjusted to a maximum of 100 mg once daily according to blood pressure response.
      • (3) Losartan potassium is not recommended for use in pediatric patients under 6 years old, in pediatric patients with glomerular filtration rate < 30 mL/min/1.73 m2, and in pediatric patients with hepatic impairment.
    • 7) If blood pressure is not controlled by monotherapy of this drug, it can be administered in combination with other hypertension drugs (e.g., low-dose diuretics). When co-administered with hydrochlorothiazide, an additive effect has been observed.

  • 2. Nephropathy in Type 2 Diabetic Patients with a history of hypertension

    The usual dose is 50 mg once daily. The dose may be increased to 100 mg once daily according to blood pressure. However, the initial dose of 25 mg is recommended for patients who are concerned about excessive blood pressure drop. This drug may be coadministeredwith other hypoglycemic agents (e.g., sulfonylureas, glitazones and glucosidase inhibitors).
